  • Dr. Todd Cohan O.D.

    Dr. Todd Cohan received his Doctor of Optometry degree from the Illinois College of Optometrists (ICO) in 2001. Using his personalized approach to vision care as the foundation, he opened his own private practice, Forsight Vision, in 2007. Dr. Cohan has always embraced new technologies to provide the most comprehensive vision care for his patients. Dr. Cohan has been treating patients with dry eye for more than 10 years. With the increased prevalence in dry eye due to our work environment, the importance of adding advanced diagnostic techniques and treatment procedures to manage dry eye became a priority to the practice. This led Dr. Cohan to develop a unique treatment facility, the Midwest Dry Eye Center, that provides the team with cutting-edge resources and modern equipment to precisely diagnose and provide relief for patients suffering from dry eye. Dr. Cohan has been invited nationally to speak to industry professionals about contact lens comfort for dry eye-related issues as well as about specialty multi-focal contact lens fitting. His passion for continuous learning drove him to become a founding member of a national study group, comprised of 12 leading optometrists across the U.S. Dr. Cohan resides in Buffalo Grove with his wife and two children. He enjoys playing tennis, exercising, attending sporting events, traveling and spending quality time with his family.

  • Dr. Jacqueline Cozzone O.D.

    Dr. Jacqueline Cozzone is excited to serve patients of all ages in the Long Grove community. After receiving her Bachelor’s degree in Biology/Pre-optometry from Augustana College, she earned her Doctorate at the Illinois College of Optometry in 2018. Her rotational experience provided her with advanced knowledge of ocular disease, contact lenses, and pediatrics. Dr. Cozzone currently resides in Palatine with her husband and dog. In her free time, she enjoys group fitness classes and spending her time outdoors.
